GP Referrals

All referrals to Dr Sardelic should be sent directly to his rooms for triaging.

Referrals can be sent by:

1. Fax - to 02 6766 8026
2. Email - to surgery@franksardelic.com
3. Via Medical Objects

If a referral is urgent it can be helpful to ring the rooms on 02 6766 7200 . If the referral is not received within a 24 hour period the staff will then ask for it to be re-sent.

If you need urgent advice, you can speak to either Dr Sardelic or his assistant, Dr Alison Harris. Please note that Dr Sardelic is in the operating theatre for 2.5 days/week, and may not be contactable.

Please note, Dr Sardelic does not perform colonoscopies / gastroscopies. He also does not see patients younger than 12.

Please ensure your referral has:

1. Accurate contact details for the patient, including address and phone numbers; if the patient has a carer, please include the carer’s contact details as well;
2. A full description of the problem;
3. The results of investigations including relevant blood results and imaging;
4. Any relevant historical imaging/ blood results/ previous operation reports;
5. Any further planned/ordered investigations - please ensure Dr Sardelic will receive a copy of those results.
